Cuando who o what es el sujeto
No se intercambia nada:
- Who had left the door open?
- What had gone wrong?
Son las preguntas de una investigación: algo ya es verdad, y preguntas qué lo produjo.
How long had
Pregunta por un tramo de tiempo que ya había corrido hasta un punto del pasado:
By the time she retired, how long had she worked there?
La respuesta lleva for: For thirty years.
Preguntas contadas por otro
En cuanto la pregunta se cuenta, el orden vuelve a ser normal:
- directa: Where had he been?
- contada: She asked where he had been.
Dónde se tuerce para un hispanohablante
«¿Dónde habías estado?» se calca como Where you had been? y falta el intercambio: el auxiliar tiene que pasar delante del sujeto.
Y en la pregunta contada el español mantiene el orden de pregunta («me preguntó dónde había estado» y «¿dónde había estado?» tienen el verbo en el mismo sitio), mientras el inglés lo cambia: She asked where he had been, no where had he been.

Had va delante del sujeto.

Cuando who hace la acción, no hay un segundo sujeto.
