Had para qualquer sujeito, depois o particípio passado do verbo.
O particípio passado é a terceira forma das tabelas de verbos: written, gone, built. Ele nunca muda com o sujeito.

O padrão
Nenhuma concordância — had é igual para todos os sujeitos:
I had gone. She had gone. They had gone.
A forma curta é ’d: I’d, you’d, he’d, we’d, they’d. Note que he’d pode ser he had ou he would — a forma depois dele diz qual. He’d gone é had; he’d go é would.
already, just, never
Eles ficam entre had e o particípio passado:
- The shop had already closed.
- He had just left when you rang.
- She had never flown before.
by the time, before, after
As molduras mais comuns deste tempo:
- By the time we arrived, they had eaten.
- She had finished before the bell rang.
- After he had spoken, the room went quiet.
Com after, o Past Perfect é opcional — a palavra já fixa a ordem —, mas nunca é errado.
Na narrativa
Uma narrativa no Past Simple pode dar um passo atrás por um instante com o Past Perfect e voltar:
He opened the letter. It had been posted three weeks earlier. He read it twice and put it away.
