by ou with
- by responde como você fez — um método, um processo, um meio de transporte: by train, by email, by hand, by asking
- with responde o que você usou — uma ferramenta física: with a hammer, with scissors
He opened it with a knife (a ferramenta) / He opened it by cutting the tape (o método).
O português usa «com» para a ferramenta — «cortou com uma faca» —, então with tem pista. O que não tem é by: «veio de ônibus» não sugere nenhuma das duas.
Note que by não leva artigo com transporte: by car, by bus, by plane — nunca by the car.
on foot
A exceção. Todo veículo leva by, mas andar leva on:
I go by bus, but today I came on foot.
Aqui o português ajuda: «a pé» também troca de preposição, exatamente como o inglês.
Outras expressões fixas com on: on purpose, on the phone, on horseback, on tiptoe.
with para sentimentos
With também introduz a maneira como alguém faz algo:
She handled it with great care. He accepted with obvious pride.
like ou as
- like compara: He works like a machine. — ele não é uma.
- as afirma um papel: She works as a translator. — é a profissão dela.
He speaks like a native quer dizer que ele não é um. He speaks as a native quereria dizer que é.
O português usa «como» para as duas, e é dessa palavra única que vem a confusão.
without
Seguida de substantivo ou de uma forma em -ing:
He left without a word. She signed it without reading it.
«Sem ler» leva infinitivo em português; without reading leva -ing.

Erros a evitar
Errado: He cut it by a knife. certo: He cut it with a knife.
With leva o instrumento. By leva o método.
Errado: She went by foot. certo: She went on foot.
Expressão fixa: on foot, ainda que todo veículo leve by.
Errado: He speaks as a native. certo: He speaks like a native.
Like compara. As afirma o que alguém realmente é.
